  • The candidate's contact information as indicated below:
    • Name
    • Location (approximate City and State is helpful)
    • Phone Number(s)
    • E-mail
    • Healthcare Discipline Type

Should the new candidate you refer be hired into and successfully complete 13 weeks of a temporary or 6 months in a permanent contract through Global Healthcare and Rehab you will receive a referral bonus of $1,000.00.
